The vowels (A, O, I) sit low on the chart so they are loud and easy to hear.
But the crisp consonants like F, S, and TH, which make words distinct (“say” vs “they”), live at the top corner, right where hearing fades first for most people.
That’s why voices start to sound mumbled.
And why you might say, “I can hear you, I just can’t understand you.”
But what’s actually causing that loss?
Inside Your Ear: Where Sound Becomes Thought
Sound travels through your ear canal, hits the eardrum, and is amplified by three tiny bones.
Those bones push against the cochlea — a spiral-shaped organ deep inside your skull.

Inside the cochlea are three fluid-filled canals, and between them is a thin membrane covered with thousands of microscopic hair-like sensors called stereocilia.
When sound vibrations move the fluid, these stereocilia bend, creating tiny electrical signals that travel along your auditory nerves to your brain.
This is how sound waves become brain waves.
It’s a stunningly complex system and it’s also incredibly fragile.
Problem #1: The Stereocilia Wear Out
Every time you hear a sound, your stereocilia bend back and forth hundreds of times a second.
Over the years, they wear out and die and unlike skin or blood cells, they never regenerate.

That’s why clarity disappears gradually over time: there are simply fewer stereocilia sending clean sound signals to your brain.

The signals from those stereocilia travel through delicate nerve fibers.
But just like any cell, those nerves wear out with age, especially because they have to fire faster for higher-pitched sounds.
That means this type of hearing loss also affects higher frequencies more than low.
The problem is also increased because you get less blood flow to your cochlea so this all combines into neural hearing loss: another reason why speech starts sounding distorted rather than just quiet.

Inside the cochlea, the vibrating membranes that move sound waves become stiffer with age.
Think of it like a drum skin becoming too tight.
At first, it’s perfectly tuned. But over time, it loses flexibility and stops transmitting fine vibrations accurately.
That means the stereocilia underneath can’t respond correctly, and speech becomes harder to separate from background noise.
